Drug Use Evaluation on Vancomycin





Abstract
By criteria of American Society of Hospital Pharmacists (ASHP) and criteria revised informally, drug use evaluation (DUE) on vancomycin, ¥³ to treat adult inpatients of Seoul National University of Hospital with was reviewed retrospectively. Total 81 charts of adult inpatients who were ordered vancomycin between June and July 1998 were reviewed but 15 cases were excluded for poor data, so the data of 66 patients were collected. Fifty nine percentage were met with criteria for the justification of use, the rest 41% were deviated from the criteria. In analyzing process indicators, appropriate initial dosage, the elements of measuring serum vancomycin level, and duration of therapy were unacceptable above 50%, especially. In the elements of complication were met with the criteria well, but this result was derived from the defect of the retrospective study. In outcome measures, negative cultures within 24 hours after discontinuation were infrequently performed among 3 elements. In revised criteria informally, percentage of the elements met criteria was increased slightly. As a result, the difference of data between 1991 and 1998 was not significant. For strict usage, formal usage guidelines for antibiotics at Seoul National University Hospital like criteria of ASHP, pharmacists joining with infection team, and ward-resident pharmacists monitoring adverse drug reactions prospectively, are needed.
